Getting connected with the Access Centre for Students with Disabilities for Services
-----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------
New students with disabilities can register with the ACSD by making an appointment to see an ACSD Advisor for an Intake Interview/Needs Assessment Interview. Students are required to provide appropriate documentation regarding their disability. This documentation will be the basis for determining eligibility for services. Referrals may be given for services that are outside the scope of the ACSD (eg. personal counseling, tutoring, assistance with study skills, etc.).
-----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------
In order to be eligible for disability-related services from the ACSD, students must satisfy the following conditions:
-----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------
Procedures for receiving services
Students new to the ACSD will meet with an ACSD Advisor for an Intake Interview.
-----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------
Review of documentation/outside evaluations
Students requesting accommodations from the ACSD must provide a copy of their physical, medical, psychological, and/or psycho-educational evaluation. The ACSD Manager and the assigned ACSD Advisor will review the documentation for adherence to ACSD criteria. Eligibility for services through the ACSD at Concordia University is dependent upon the nature of the disability and its impact on learning styles and needs.
Please note that a person might meet eligibility requirements of rehabilitation agencies; however, he/she may not meet eligibility at Concordia University. One of the reasons that the University developed these guidelines is for consistency throughout the institution. These guidelines are fairly consistent with those used by agencies administering standardized assessments. The ultimate decision as to eligibility for services at Concordia University is a judgment that must be made by the Manager of the Access Centre for Students with Disabilities based upon the guidelines developed for each type of disability.
Once a student is fully registered with ACSD the determining documentation will be placed in the student’s confidential file.

Confidentiality and Student files
Confidentiality of personal information is an important component of the ACSD. Personal information provided to the ACSD is protected by virtue of the applicable laws and the Concordia University Policy Concerning the Protection of Personal Information
The ACSD maintains student files for five years after the last year in which the student was active at Concordia University. After that time, the file is destroyed.
At times it is necessary for staff at the ACSD to speak with Professors and other University personnel regarding an ACSD registered student. Such internal communication is permitted and necessary to ensure the efficiency and quality of ACSD’s services.
